Easy sight upgrade not so easy

I've enjoyed my 586 for a couple of decades. I'm retired now, and able to do a little more shooting. My other S&Ws have white-outline rear sights, but my 586 doesn't. So, I thought I would try and rectify that. I'm fairly handy with tools having been in aircraft maintenance for twenty years, and being a copier/printer technician for another twenty.
I purchased the factory upgrade kit from Midway. I watched a few YouTube videos on how to replace the blade and today I thought I would give it a go. I fashioned a spanner for the nut using a small screwdriver and Dremel tool. I got everything apart just fine, being careful not to lose the spring and plunger. I put it all back together, tightened it up, backed the screw off 1/4 turn and staked the screw into the nut. Everything seemed to work perfectly, until...
My sight blade was in the middle and I decided I'd turn it both ways just to make sure it moved okay. I turned a little, the screw clicked like it was supposed to but it didn't seem like the blade moved. I checked the nut and it was staked good and was spinning with the screw. Yet, no matter how much I turn the screw the blade does not move.
All I can figure is that somehow the blade/screw threads are stripped, but I can't figure how that could have happened. I screwed the blade onto the screw all the way before installation, and even applied a tiny drop of gun oil to it so it would always turn freely. Then, I backed it off to the middle and slid it in. While turning the screw I can push on the blade in either direction and there is zero movement. I'd think if it was stripped, it would move a little with pressure applied. But, it doesn't.
Since the blade is in the middle, maybe it will shoot okay and not need any windage adjustment. Yeah...right! I'm sure this kit is ruined. But, before I buy another and try again, have any of you ran into this problem? How do I prevent the same thing from happening again?

The only thing you can do at this point is take the blade back out and examine the parts. You should be able to unscrew the nut off the screw by using your nut tool to hold the nut while you back out the screw. Take the screw out of the sight blade and examine the threads on both. One or the other is probably stripped out. Once you find the problem, you'll know where to go from there.

Normally this is a straightforward process. Your problem is highly unusual.

I guess that's all I really can do. I just can't figure how it got stripped. It worked fine before I put the nut on and staked it.
