A belt holster has belt loops/slots designed to retain the holster to a belt and therefore cannot be removed without removing the belt. IWB holsters have clips which may or may not be worn with a belt. Some of the clips are designed to not engage the belt at all but rather the belt, if worn, goes over the clip--The clip engages the top seam of the pant waist.
I would take it at face value until the Texas AG offers a ruling. I certainly don't want to test it. I read it as a strict definition as a holster designed to be worn on a belt. It's no different than the phrase states that a shoulder holster is the only other option. While there are different styles of shoulder holsters, the design is definitive and well known.
